Continuum of Care

Harford Community Action Agency's Continuum of Care program in Edgewood, Maryland, supports sheltered and unsheltered homeless individuals by providing housing and essential services for a transition to permanent housing and long-term stability. Guided by 24 CFR part 578.1, the program aims to end homelessness through community-wide efforts, rapid re-housing, and reducing the trauma of homelessness. By collaborating with nonprofit providers, states, and local governments, the program promotes access to mainstream resources and fosters self-sufficiency among homeless individuals and families.
