InnovAge PACE has an extensive provider network in each community we serve.

Medical specialists in our provider network treat many conditions. PACE participants have access to dentures, hearing aids and eyeglasses at no cost if determined necessary by the interdisciplinary team (IDT). These benefits are not always included in other senior healthcare programs. Other conditions treated in PACE InnovAge doctor visits senior PACE participantinclude but are not limited to:

Cancer

Dementia, including Alzheimer’s disease

Depression and other mental health conditions

Diabetes

Emphysema

Heart disease

Kidney disease

If the care team determines you need treatment from a medical specialist, InnovAge will coordinate appointments and care at no additional cost to participants.

Please note that InnovAge PACE participants must receive all needed healthcare, including primary care and specialist physician services (other than emergency services) from InnovAge or a person or company authorized by InnovAge. InnovAge PACE participants may be fully and personally liable for the costs of any out-of-network services.
