Disenrollment Rights & Responsibilities


Participants have the right to disenroll from PACE through a smooth process coordinated with Medicare and Medicaid. Reasonable notice is provided, and all PACE services remain available until the official disenrollment date. Participants are responsible for continued use of services and any applicable premiums during this time. After disenrollment, PACE helps transfer medical records and coordinates with other programs to ensure a seamless transition of care.
