Healthcare providers can refer eligible older adults to Rocky Mountain PACE, a program specifically designed for individuals aged 55 and older residing within designated service areas. Ideal candidates are those who require support with activities of daily living (ADLs) or need supervision due to physical, cognitive, or behavioral health conditions. Additionally, participants must be determined by the State of Colorado to meet the eligibility criteria for nursing home-level care.
