Disenrollment Rights & Responsibilities

Rocky Mountain PACE participants have the right to disenroll from the program at any time, with disenrollment taking effect on the last day of the month. The organization is required to follow Medicare and Medicaid procedures to ensure a timely and coordinated disenrollment process, including notifying participants in advance and aligning termination dates for those eligible for both programs. Until the disenrollment is effective, participants must continue using PACE services and remain responsible for any applicable premiums, while Rocky Mountain PACE continues to provide all necessary care. To support a smooth transition, the organization facilitates referrals and transfers medical records to new providers within 30 days and collaborates with CMS and state agencies to help reinstate participants in other Medicare and Medicaid programs.
